The object of study: 377 patients with chronic lymphocytic leukemia (CLL), including 104 ionizing radiation (IR) exposed patients due to Chornobyl NPP catastrophe, and 327 individuals without oncohematological pathology (136 sufferers of the Chornobyl NPP catastrophe and 191 IR-non-exposed persons). Methods: the determination of CD38, PDE4D, interferon-gamma gene's polymorphism by polymerase chain reaction and restriction of products, clinical, statistical. The main results: the distributions of polymorphisms rs6449182 of CD38 gene, rs966221 of PDE4D gene, and rs2430561 of interferon-gamma gene in the control group did not differ depend on radiation anamnesis of persons and corresponded to the others groups of Caucasians. No differences were revealed among IR-exposed and IR non-exposed CLL patients. The impact of GG genotype of CD38 gene on the risk of CLL development was found: Odds ratio (OR) = 2.05 (95% confidence interval 1.211-3.48; p=0.001). An association between GG genotype of CD38 and dyslipidemia was found in the control group. All studied polymorphisms had influence on structure of leukemic cell's B-cell receptor. Some associations of rs966221 of PDE4D gene's genotypes with clinical features of CLL were revealed: low efficacy of second line therapy (p=0.03) and high frequency of cytopenias after chemotherapy (p=0.073) in carriers of TT genotype; high frequency of bulky disease in carriers of CC genotype (p=0.035). CLL patients with GG genotype of CD38 gene were more likely to have higher initial leukocytosis (p=0.018), splenomegaly (p=0.06), higher risk of Richter transformation (p=0.001) and shorter overall survival (median 56 months) comparing to carriers of CC and CG genotypes.
