It is probed - 125 patients with complicated chronic pancreatitis, 142 experimental animals. Research purpose: improvement of results of surgical treatment of patients complicated chronic pancreatitis on the basis of study of processes of inflammation, fibrosis and regeneration of pancreas parenchyma and ground of expedience of application of new technologies of their surgical correction. Research methods - clinical, experimental, biochemical, instrumental, morphological, immunological, microbiological and statistical. It is set that weight of complications of chronic pancreatitis depends on the term of disease and degree of fibrotic alteration of pancreas which at chronic pancreatitis takes a place from destruction or atrophy of ferrous fabric depending on etiologic factors. Biochemical, immunological indexes and features of instrumental researches which allow to set activity of inflammation and degree of fibrotic transformation of pancreas at complicated chronic pancreatitis are certain. It is proved that at the choice of method of treatment of complicated chronic pancreatitis it is necessary to pay a regard to activity of inflammation and degree of fibrotic of pancreas. At the surgical correction of complications of chronic pancreatitis on a background active inflammation conservative therapy and implementation of minimalinvasion palliative surgical interferences is shown final correctings surgical interferences must be executed on a background nonactive inflammation and taking into account the degree of fibrotic of pancreas. The algorithm of diagnostic and medical tactic, which allows to set character of complications of chronic pancreatitis, set a kind and term of treatment and concretely necessary operation taking into account activity of inflammation and degree of fibrotic of pancreas, is developed, that enables to reduce the number of postoperative complications and improve quality of life of patients.
