Purpose - to increase the effectiveness radiotherapy and chemotherapy treatment of locally common forms of cervical cancer by developing new methods of brachytherapy sources using high activity and application chemo radiomodifiers during combined radiotherapy. Research Methods and apparatus: clinical, laboratory , X-ray diagnostics , dosimetry . Simulator session irradiation - "SimView" 3000 (Siemens, Germany) TERATRON , AGAT- VU (MDR), GUNE SOURCE (HDR), ultrasound machine "Aloca" - FLEXUS, apparatus for magnetic resonance imaging studies ( MRI ) " Magnetom - Vision-Plus-System "(Siemens), universal dosimeter PTW Freiburg "Unidose E ". The main groups were 67 patients with cervical CF who brachytherapy (BT) was performed on the machine Gyne Source of high dose rate radiation (HDR) modes in a single focal dose ( SFD ) on December 5 t A 2 times per week to a total focal dose (TFD) 35-40 Gy in 7 - 8 fractions and SFD 7 Gy and so on 1 day a week to SFD 28-35 Gy in 4 - 5 fractions. The control group consisted of 39 patients who performed on the BT device AGAT- VU with an average dose rate radiation (MDR), SFD on dot A SFD 8 Gy 1 every 6 - 7 days to SFD 40-48 Gy in 5 - 6 fractions . All patients received chemotherapy - Ftorafur radiomodifying at a dose of 800 mg per day, per os. A program of quality control by BT and characteristics of high- dose gamma radiation Co60 on the machine Gyne Sourse . The stages and sequence topometry preparation patients to HDR SE CC BT device to Gyne Sourse . The optimum fractionation regimes of HDR BT in radiation treatment of cervical MP, indications and contraindications to their use. The established sequence studies ( clinical, sonographic , radiological ) to monitor the effectiveness of chemoradiotherapy in patients with cervical cancer using MP HDR BT. Methods HDR BT improves immediate results chemoradiotherapy ( CRT ) in patients with cervical cancer MP : objective tumor response (complete + PR) in the application of the regime SFD 5 Gy increased by 16 % when using Mode SFD 7 Gy - 22 % compared with the control group. The absence of excessive toxicity developed techniques HDR BT provided quality before irradiation preparation and application of adequate prevention evidence of their effectiveness. Analysis of long-term results of patients with CRT SE CC also shows the effectiveness of the developed techniques HDR BT -measures three-year survival of patients in the application mode of SFD 5 Gy increased by 13,9 %, and the use of regime SFD 7 Gy - 18,6 % compared with the control group ( MDR BT) .
