Subject of study. During the post-thrombotic disease (PTD) and deep vein thrombosis (DVT) of the lower limbs. Purpose of research: to increase the effectiveness of rehabilitation of sick and disabled as a result of post-thrombotic disease of the lower limbs by identifying the leading causes of disability and to develop an optimal technology for their medical and social rehabilitation with the use of modern surgery. Research methods and apparatus. 1.Clinical-experimental: 1)clinical and functional (examination the subfascial pressure on the tibia; electroneuromyography of the lower extremities, color duplex scanning, computer venography of the systen of vena cava inferior. 2)biochemical studies: D-dimer conduct test, examination the state of the coagulation and fibrinolytic systems, stabilization plasma potential in the regional and systemic vein blood flow to the definition of the gradients of these indicators, the definition of B-fibrinogen , protein C, globulin fraction of blood. 2.Methods of processing results of the study: 1)systematization of objects (association of sick and disabled as a result of PTD in homogeneous groups depending on the severity of chronic venous insufficiency period after acute DVT, duration of disability, to further study in each group. 2)statistical methods. To carry out surgical interventions on patients by category of research was a set of tools for endoscopic procedures, in particular subfascial dissection of perforating veins, for venectomy (catheter for venextractsion, knives epifascial venectomy) set Mueller-Varadi for minimally invasive venectomy, elektrodermatom EDVP-100, dermatome AНD-60 with manual transmission, etc. Scientific novelty. On the basis of biochemical, haemostatic, electrophysiological, sonologic methods established leading pathogenetic aspects of DVT, the progression of PTD and their possible correction for the sensitivity and specificity of selected diagnostic methods. First by comparing the results of duplex veins angioscaning of the lower limbs, computer venography, in-depth study of systems of indicators of fibrinolysis, coagulation developed clear indications for minimally invasive surgical procedures in patients and disabled due to PTD. The improved technology of complex medical and social rehabilitation pathogenetically substantiated application of minimally invasive surgical procedures. Requires high efficiency due to comprehensive rehabilitation of PTD through the use of surgical interventions and individualized approach to conservative treatment. Practical importance. Improving the efficiency of medical and social rehabilitation of sick and disabled due to PTD of the lower extremities due to the differential application of modern features restorative treatment using advanced surgical techniques will reduce the rates of primary and accumulated disability in Ukraine as a result of this disease. Implementation of research results will allow physicians and health care facilities of medical and social expert committee (MSEC) more perfect to form and perform medical section individual rehabilitation program, due to this will be improved results and rehabilitation of patients disabled due to PTD. Practical application development performed research can achieve the following medico-social and socio-economic effects. Medical and social effects of improved criteria for medical and social examination of sick and disabled as a result of post-thrombotic disease. Proposed pathogenetic informed surgical treatment in complex rehabilitation and identify measures of conservative therapy can improve the results of rehabilitation treatment (to reduce the number of unsatisfactory results of surgical treatment of post-thrombotic disease to 4% of patients treated). Socio-economic: the use and implementation of the work of the MSEC developed the basic program of medical and social rehabilitation will reduce the accumulated indicators of disability due to post-thrombotic disease and improve quality of life and care for the disabled. This will reduce the economic costs of rehabilitation treatment and payment of state subsidies in this cohort of persons with disabilities.
