Objective: To reduce the incidence of early reproductive losses from a study of pathogenetic mechanisms damage the reproductive system and develop an algorithm for diagnosis and treatment of complications of pregnancy and preconception preparation of women with early pregnancy loss as a result of clinical, sonographic, endoscopic, endocrinological, immunological, morphological, immunohystochemical and genetic research. The study involved 434 pregnant women at 4-12 weeks of gestation: 220 - with threatening miscarriage, 84 women with missed abortion, 21 patient with spontaneous miscarriage, 89 women with tubal pregnancy, 20 pregnant women with singleton pregnancy that normally develops. On the basis of these studies was found that the early pregnancy losses have different etiologic factors and pathogenic mechanisms . Genetic disorders of embryons occurred in 43,5 % of women with missed abortion and 46.7 % of women with spontaneous abortion, the most severe forms of chromosomal disbalances found in the earlier stages of pregnancy. The most significant cause of early reproductive loss is breach of uterine hemodynamic and formation of placental hemodynamic. Established morphological and immunohistochemical criteria for changing the structure of the chorion, that indicated disturbance of proliferation and apoptosis shown by decreased expression of Ki-67 and of protein bcl-2 in decidual membrane, and leads to inhibition of gestational vascularisation of decidua and subsequent death of the embryo. Available changes occur to the damage of immune balance and tolerance on all levels: the systematic - the cytokine disbalance towards production of proinflammatory cytokines, straining of specific protection and reduction of nonspecific protection; the local - manifested in the reduction of secretory immunoglobulin A and lysozyme, as nonspecific factor that protect mucous membranes; the cells - increasing of natural killer cells (NK- cells) in decidua and in the chorion. Progesterone deficiency of varying degrees of severity was found in almost every survey of patients, decrease of expression of estrogen and progesterone receptors in decidua indicates the pathogenic role of hormonal changes in the mechanisms of reproductive losses. Increased microbial contamination of pathogenic microorganisms of high titres, the presence of inflammatory changes according to morphological studies, which occurs in 75 % of women with abnormal pregnancy is a predictor of pregnancy interruption. Studies show that women with early pregnancy loss are at high risk for the occurrence of benign breast dysplasia, that depends on the type of concomitant thyroid pathology. Established criteria for early diagnosis of complications of pregnancy by using of color Doppler that can detect violations at preclinical stage. A new system of diagnosis, treatment, prevention and rehabilitation of women with various forms of early pregnancy losses was developed, which improved the course and outcome of pregnancy for mother and fetus.
