Ultrastructural examination of myocardial express-necropsies of 38 patients who died from acute myocardial infarction (AMI) and 32 myocardial biopsies obtained during coronary artery bypass surgery or ventriculoplasty of chronic aneurism wall was performed. It was revealed that AH causes microvascular remodeling and narrowing as well as cardiomyocyte (CMC) hypertrophy and hibernation, leading to heart failure development. DM causes diabetic microangiopathy and cardiomyopathy in the result of microvessel reduction and CMC hibernation and dysfunction. The development of AMI in the patients suffering from AH and DM stimulates CMC necrosis and apoptosis in intact and near necrotic zones of the left ventricle, causing HF progression. Myocardial revascularization and prescription of cardioprotective drugs with antiapoptotic effect in AMI cases and comorbidity may prevent the severity of the heart failure and systolic dysfunction of the left ventricle in AH and DM patients.
