The purpose of the research: to determine the risk factors of slowly progressive LADA development, to improve the algorithms of it's diagnosis for timely verification of this form of diabetes by virtue of the study of clinical manifestations features, cellular and humoral immunity, genetic markers. Research methods - clinical, biochemical, hormonal, immuno-enzymatic, instrumental, genetic, statistical.The method of determining the risk group of LADA possible development on the stage of disease manifestation has been provided. It is necessary to define at least of two kinds of antibodies for LADA verification. Antibodies to glutamic acid decarboxylase and tyrosine phosphatase are the most conclusive. LADA patients show the lipid disorders that are more inherent in such that observed in type 2 DM patients. The data of the high prevalence of autoimmune thyroiditis in LADA patients justify the necessity of focus examination of the thyroid gland state in patients with this form of DM. LADA patients reveal the pronounced microvascular complications in a short period of disease. Significant decrease in the content of T - suppressor / cytotoxic lymphocytes and increased immunoreactive index without the probable difference in the content of T-helper cells in LADA patients testify of the inhibition of suppressive mechanisms. It may be due to their participation in the development of the local autoimmune inflammation (insulitis) in the presence of LADA. High levels of IL-2, IL-4, IL-8, IL-10 and tumor necrosis factor-alpha were determined in these patients. The family savings and molecular genetic studies confirm the genetic autonomy of LADA. The influence of cross-breeding degree on the propensity to LADA has been shown. The association of S1858T PTPN22 gene polymorphism and 49A / G gene CTLA4 with LADA has been defined.
