The content of intracellular NF-kB has been studied among CD40+ cells populations under the influence of pioglitazone in patients with metabolic syndrome and coronary heart disease. The data on blockade of NF-kB activation has been obtained under the action of endogenous proinflammatory cytokines (by the example of CD40+ mononuclear cells of peripheral blood) against the background of metformin inclusion in the complex treatment of patients with metabolic syndrome and coronary heart disease. The reduction of NF-kB expression levels and apoptosis processes of CD64+ cells has been determined in the mononuclear cells suspensions of peripheral blood under the influence of PPAR gamma activator. The distribution of genotypes frequencies and alleles of PPARgamma2 gene has been investigated with due consideration of patient's lipid metabolism changes. The high therapeutic efficacy of metformin has been detected in patients with coronary heart disease against the background of type 2 diabetes mellitus or metabolic syndrome who have the genotype of Pro / Pro PPARgamma2 gene.
