Object of study - 31 patients with bone sarcomas high grade of malignancy of them - 20 nonmetastatic forms , 11 - who developed lung metastases. The purpose of research - to increase the effectiveness of treatment of patients with primary malignant tumors of bone and metastatic relapse by developing new and improving existing methods and neoadyuvant adyuvant chemotherapy. Research methods and apparatus: clinical, radiological, morphological , immunohistochemical, statistical , computer scanner Somatom 4 Plus Siemens, Germany ; Magnetic Resonance Imaging Magnetom company Siemens; angiograph " Televiks "; ultrasonic diagnostic apparatus US 90,301,710 ; microscope Nikon Esclipe 80i. Based on a retrospective analysis of patients developed distant metastases developed a diagnostic algorithm for selection of high-risk patients based on immunohistochemical markers forecast data, as well as the location and spread of the tumor. The technique neoadyuvantnoy chemotherapy for patients with high risk of metastasis , which includes 3 components ( 12 g/m2 methotrexate , doxorubicin 75 mg/m2 , cisplatin 120 mg/m2) . Feature of the scheme is the use of intra-arterial administration of cisplatin , the assessment of the dynamics of inhibition of tumor neovascularization , which is the main component in the characterization of therapeutic pathomorphism. Developed second-line chemotherapy for the treatment of metastatic bone sarcomas - a combination of 3 - drugs : carboplatin 450 mg/m2, 300 mg/m2 of etoposide and ifosfamide 8 g/m2.
