Benefits of early combined use of enteral and parenteral nutrition in patients with severe TBI with aim of correction syndrome of hypercatabolizm-hypermethabolism in patients with prolonged coma period improves the clinical and laboratory parameters, limits the intensity of catabolic reactions , reduces infectious complications and organ disorders, indicators of late mortality. In infants was found dominance of system septic complications over local unlike complications in adults. Developed and implemented algorithms for prediction of septic complications, schemes and algorithm of prevention of septic complecations using antibiotics in infants in abdominal surgery. This makes it possible to reduce the incidence of infectious complications , mortality , disability in these patients and the costs of treatment and hospital stay. Using of preventive therapy with recombinant interleukin type 2 in patients with multiple trauma promotes SIRS regression: reduction of markers of inflammation (leukocytosis and neutrophilia), common variables (normalization of glicemia), regression of signs of organ dysfunction ( reduction of urea and fibrinogen, increasing the total blood protein) and provides prevention of organ failure syndrome. Proved feasibility of recombinant IL-2 as a method of preventive therapy in early postshock period, to prevent the occurrence of SIRS with development of early ifection complications of traumatic disease in patients with multiple trauma. Developed and improved diagnostic criteria of acute hepatic dysfunction as a component of the syndrome of poliorganic failure and assessing of severity of hepatic dysfunction. Based on these criteria (assessment of cytolysis, synthetic, pigment and detoxic function of liver) we first proved feasibility of separation of acute liver dysfunction in three successive stages: compensated, subcompensated and decompensated and scientifically confirmed selection of the most appropriate technologies of efferent therapy depending on the severity of acute liver disfunction. Propose criteria of diagnostic of acute liver disfunction and assessment of its severity are available for most hospitals.
