M.I.Sitenko popup.description2 Object of a study - restoration of the anterior supporting complex of the cervical functional spinal units in cases of surgical treatment of their diseases and injuries. Purpose - to ensure an optimal recovery of the anterior supporting structures and sagittal contour of the cervical functional spinal units in cases of anterior interbody fusion in surgical treatment of cervical spine diseases and injuries. Methods and equipment - clinical, radiological, instrumental, mathematical modeling, finite element method, the information and statistical. X-ray machine, ultrasound machine Sonoline-G50 (Siemens, Germany). Theoretical and practical results: There were defined an efficiency and disadvantages, as well as a number of complications and mistakes while anterior interbody fusion using by means of a variety of techniques and tools. It was substantiated and developed a system of radiological assessment for multilane spatial position of the elements in system "cervical functional spinal units - fixing constructs" after anterior interbody fusion which allows to determine precisely the dynamics of their relative position in the stabilized segments and correlation with clinical symptoms. There were developed and refined 5 techniques of anterior interbody fusion and external immobilization with cervical orthoses as well as items for their implementation the effectiveness of which compared to known methods was proved experimentally and confirmed clinically. There were built and improved 2 multilevel mathematical models of the cervical spine in which with finite element analysis there was firstly identified some patterns of changes in the stress-strain state after anterior interbody fusion in systems " cervical functional spinal units - implants - orthoses" depending on the design of fixing devices, items of external immobilization as well as techniques of their application. An inexpediency of using of vertical cylindrical mesh cages without cervical plates was proved which provokes their prolapse onto the vertebral bodies. It was determined that the result of anterior interbody fusion using plates primarily depends on peculiarities of the contact between interbody support and vertebral bodies. It was revealed that the main result of the application of dynamic constructs is clinically insignificant decreasing in the value of the reconstructed segmental cervical sagittal contour on a background of optimization and shortening interbody fusion. Knowledge on the relationship of features in structural and functional state of the vertebral arteries with clinical symptoms which allows to establish the potential adverse types of post-traumatic deformities accompanied by disturbances of blood flow in the vertebrobasilar basin. The novelty - for the first time according to clinical and radiological data some peculiarities and benefits of using developed and improved techniques for anterior interbody fusion by means of hybrid metal-ceramic implants and cervical plates in comparison with known methods were revealed. The essence of the process of dynamization during the formation of interbody fusion in the postoperative period due to the biplanar rotational repositioning of the cranial pair of screws in the sagittal and frontal plane was firstly elicited. Effectiveness of implementation - The proposed methods allow to achieve considerable stability of fixation and preservation of fixing properties of constructs. Surgical treatment of patients according to the schemes developed allows to achieve excellent results in 78.6% of cases, and neutralize the number of poor results against the backdrop of significant reduction in the indications for surgical treatment on 19.8%.
