Topchiy, Registration Date 23-01-2015 Organization Government institution "L.T. Malaya Therapy Institute National Academy of Medical Sciences of Ukraine" popup.description2 The object of study - 236 patients with chronic kidney disease (CKD), chronic glomerulonephritis (HGN) - 78 and chronic pyelonephritis (CPN) - 158, 162 patients with arterial hypertension (AH) and 20 healthy donors. In the experiment material of study were 40 white mongrel rats weighing 200-300 g, aged 4 month old. Methods: content sCD40, MCP-1, VE-cadherin soluble serum samples were determined with ELISA, activity of parooksanaza 1 (PON1) - method using paraokson; content metabolites of nitric oxide - the reaction Gris; expression of adhesion molecules of white blood cells evaluated by immunohistochemical method; content of malondialdehyde - used reaction with tiobarbiturova acid; sulfhydryl groups - reaction tioldysulfid exchange; study of cardiac and kidney hemodynamics used US; quantitative analysis of circulating endothelial cells in plasma was performed by methodHladovec J .; experiment histochemical study of rat kidney preparations stained with hematoxylin-eosin. At autopsy material to identify features of immune cell responses in areas of inflammation used primary monoclonal antibodies (PMA) company DAKO (Denmark), Rady - to - Use. Immunohistochemical method detected the expression of T- and B-cell clusters of differentiation (CD3, CD20), plasma cell marker (CD38), macrophage marker (CD68). Features endothelisationof vessels intimastudied with expression of endothelial cell marker (CD31 JC 70a), tendency to vascularization was assessed by vascular endothelial growth factor (VEGF (VG1)). As a marker of apoptosis bcl-2 was used. The use in the treatment of patients with CKD L-arginine reduces the content of sCD40 and excessive VE-cadherin production, increases the activity paraoksanazy-1, improves the integrity of the endothelium and its NO-producing ability, indicating that the optimization of the immune response and antioxidant properties of the endothelium on the background improve its function. Additional appointment kardioarhinin for patients with CKD improves myocardial diastolic function on the background of improved functional state of the endothelium. Morphological study of vascular endotheliumof kidney, heart and aorta showed that in the majority vascular intima had deep lesions, manifested varying degrees of disruption of the endothelial lining of blood vessels and impaired structural and functional organization of endothelial subendotelial layer of the basement membrane.
