A significant rate of depopulation of the Kherson region due to the consistently high negative natural and mechanical growth. We found that when conducting population genetic studies of population of the Kherson region outside of an elementary population should be considered the entire area (index of endogamy 0,54-0,81). According to the index of endogamy and the level of inter-ethnic marriages between populations revealed significant differences. The General trend is to increase the level of endogamy, the increase in the share of interethnic marriages in the whole area (from 30.5 to 32.4%) and rural councils (from 30.9 to 35.7%), which can significantly affect their genetic structure. The criterion of an elementary population in the district is only responsible Gornostaevskiy (share endogamic marriages 57,0%) and all district centres, except the city of Berislav (index endogamia 0,50-0,62), with relatively stable for 54 years the level of inter-ethnic marriages (30,1-29,1 %). For village elementary populatino unit is the whole area (index of endogamy 0,64-0,80), except the villages of Nyzhni Sirohozy and Vysokopillya areas with a sufficiently high level of population migration (endogamy index of 0.48 and 0.46, respectively). It is shown that the regional populations with different genetic and demographic structure differ in the frequency of early spontaneous abortion. With the decline in the risk of reproductive losses in populations of Kherson region observed a statistically significant increase in the frequency of congenital malformations in newborns. With the decline of the resident population of the Kherson region, the decline in the birth rate there was a significant increase in the population frequency of congenital malformations in the cohort of newborns (from 24.2‰ in 2000 to a total of 35.1‰ in 2012). This trend is observed in districts of the region (from 22.7‰ to 29.9‰) and in the regional centre (of 23.2‰ to 46.1‰). In the Kherson area population frequency of a slit lip and/or palate, polydactyly, down syndrome was significantly higher compared with the corresponding data registers EUROCAT in Ukraine and in Europe. However, in Gornostaevskiy district that meets the criterion of an elementary population, on the background of decreasing share of interethnic marriages and increasing the level of endogamy for the past 13 years stated relative reduction in prevalence of birth defects, stillbirths, spontaneous miscarriages, which may be attributed to the formation of adaptive complexes of genes that counteract the effect of genetic drift.
