Romodanov NAMS of Ukraine'' popup.description2 On the basis of a retrospective cohort study in parallel groups at the 723 patients with intracranial hemorrhage due to rupture of arterial aneurysms (AA) of brain vessels it was found that endovascular (EV) treatment with embolization of aneurysm by coils remains a priority method of choice for AA of brain regardless of variants of clinical flow of disease. One of the major drawbacks of the EV treatment of AA using embolization by coils is aneurysm recanalization which is observed in 18.7% - 20% and led to the need for reoperation in 12% of cases. The factors that increase the probability of AA recanalization were revealed: AA gap, complicated with local vasospasm of affected segment of cerebral artery; a massive subarachnoid hemorrhage of intracerebral hematoma with the accession of the phenomena of dislocation syndrome and acute violation of liquor circulation due to rupture of brain AA. The anatomical factors that increase the probability of AA recanalization were revealed: wide (in comparative relation to the body) neck; the presence of atherosclerotic lesions of AA neck, a large diameter of the aneurysm (> 7 mm) and aneurysm rupture in anamnesis. The system predictors of recanalization of aneurysms embolized by spirals were identified: arterial hypertension and chronic cerebral ischemia of different genesis in anamnesis. Determination of neuromarker of brain tissue destruction - protein S100B, combined with neuropsychological testing, expanded understanding of the pathogenetic aspects of chronic brain ischemia and predicting the effectiveness of remedial measures, including EV embolization of brain AA by spirals in cases of a combination of AA with occlusive-stenotic lesions of brachiocephalic and cerebral arteries of trunk type. Possibilities of surgical correction of probable recanalization of brain AA when performing EV surgery were determined. The role of microsurgical techniques of brain AA devascularization in prognostically unfavorable cases of AA recanalization was established. A mathematical model for predicting the natural course of brain AA based on model of a periodic change in the number of antagonistic factors (V.Volterra -A.Lotky) was created.
