Objective: To examine and assess short- and long-term effects of low-dose ionizing radiation and psychosocial factors on the development of main forms of nontumor diseases among the Chornobyl victims, to justify medical-and-social care. Methods: epidemiological, medical-and-statistical, simulation; clinical. Results. Using SRU data, we estimated rates for the cohorts of cleanup workers, evacuees and inhabitants of contaminated areas (68145, 7589 and 98000 persons, respectively) and evaluated difference between level and structure of incidence depending on age at the time of exposure, dose, sex and time period since exposure. Within first 6 years after the Chornobyl accident an increase in morbidity of mental and behavioral disorders, disorders of the autonomic nervous system and hypertension was observed; during 12-21 postaccidental years growth of chronic circulatory, respiratory, digestive, genitourinary and musculoskeletal diseases was reported; in the late period, (22-26 years) decline in nontumor incidence was revealed. For some nontumor diseases statistically significant dose-dependent effects were determined among cleanup workers with whole body external doses in the range 0.05-0.99 Gy; among evacuees with internal doses from 131І in the range 0.3-0.75 Gy; among inhabitants of contaminated areas with cumulative doses in the range 0.05-0.4 Sv. Risk-analysis on CER data in cohorts of cleanup workers 1986-1987 and evacuees (7308 and 2652 persons, respectively) has revealed that age (ageing) and psychosocial factors are most important in the development of the majority of diseases under study, especially, ischemic heart disease and cerebrovascular diseases; the contribution of radiation factor to the structure of statistically significant risks for these diseases does not exceed 10 %. Sampling physical examinations in 2013-2015 have revealed peculiarities of the state of mind, behavior and nervous system in cleanup workers and evacuees (312 and 128 persons, respectively), and also impact of radiation and nonradiation factors on the development of nontumor pathology. Based on the data of sampling questioning (2013-2015) of 235 cleanup workers and 258 evacuees, we have identified factors that contribute to chronic psychosocial stress among persons affected by Chornobyl accident. Mathematical model was developed and used to calculate forecast of circulatory disease prevalence. A concept for development of nontumor pathology in a radiological emergency was worked out and proper measures to improve psychosocial rehabilitation of victims were justified.
