Objective: To study the hormonal and metabolic disorders in patients with T2D according to the phenotypic features and develop pharmacological approaches to the treatment of patients. Methods: clinical, laboratory, statistics. Theoretical and practical results. On the base of the study of hormonal and metabolic parameters in T2D patients, were particularly isolated and characterized phenotypes inherent for type 2 diabetes, the most important of which were the "type 2 diabetes with obesity" and "non-obese type 2". In patients with the phenotype of obesity, revealed high activity of the 11b-GSD1, increase in postprandial blood glucose, HbA1c, levels of C-peptide, triglycerides, increase of LDL and VLDL, decreased HDL, increased levels of cortisol while reducing the level of DHEA-S, the increase of saturated fatty acids amount in triglycerides, reduction of osteocalcin, strengthening the negative psychosomatic, high levels of uricemia, insulinemia and the degree of insulin resistance (IR), a higher fractional excretion of uric acid, reducing the number of CAG repeats in the gene-androgen receptor, a decrease in testosterone levels in men while women have increased this levels.In patients with non-obese phenotype, observed a high urinary excretion of tetragidrokortizon, lower levels of uricemia, insulinemia and the degree of IR, as well as triglycerides and LDL and decreasing HDL, low fractional excretion of urate and purine salvage index as compared with those in patients with obesity. The results allowed to argument suitability of visceral adiposity index for revealing disorders of the fatty acid spectrum of serum TG in patients with type 2. The approaches to the distribution of T2M patients into different phenotypes were developed and introduced in public health practice. Effectively implement: improving the efficiency of diagnosis and treatment of type 2 diabetes with different phenotypes. Advantages over existing analogues: accounting of phenotypic features in patients with type 2 diabetes will improve the effectiveness of treatment. Application: endocrinology.
