This algorithm starts with the detailed analysis of the clinical-and-hematological parameters (hemoglobin level, leukocyte and platelet counts) and morphocytological and cytochemical evaluation of the cells of peripheral blood and bone marrow (the detection of dysplasia signs in the cells of various hematopoietic lineages, the strong cytochemical reactions in the cells of hystiocytic-macrophage lineage). The increased anemia and thrombocytopenia with accompanying presence of blast cells in the peripheral blood (from 2-4% to 5-19%) and bone marrow (from 5-9% to 10-19%) are the basis for identifying sush as MDS-EB-1 and MDS-EB-2. The detection of the miltilineage dysplasia and more than 19% of myeloperoxidase-positive blast cells expressing the antigens of myeloid lineage (CD34, CD7, CD117, CD13, CD33) allows one to verify the stage of MDS transformation into acute myeloid leukemia classified as the secondary myeloid leukemia according to the requirements of the modernized (2016) WHO classification.
