Objective: to assess the features of coronary heart disease development and progression in clean-up staff of the Chernobyl nuclear power plant (ChNPP) that based on studying the interaction of radiation and non-radiation risk factors, gene PDE4D polymorphism rs966221 and polymorphism rs1333049 of gene in chromosome 9p21, and circulating endothelial progenitor cells analysis.Results. By Cox regression method it was revealed that the fact to be a clean-up worker together with tobacco smoking and obesity had the significant impact on risk of CHD onset. The risk of CHD onset in CUS was in 3.9 times higher than in non-irradiated men. Tobacco smoking and obesity had a three times less risk such as 1.37 and 1.33 respectively. The genotype TT carriage increased the risk of myocardial infarction (MI) onset in 2.87 times for CUS of any age in comparison with genotype CC and CT of gene PDE4D. For control group this risk valid in people being older 60. CHD course was determined by stenocardia functional class (FC) and stage of heart failure (HF). The complex of factors as hypertension heart disease (HHD), diabetes mellitus, MI in anamnesis, arrhythmia, tobacco smoking and genotype TT carriage significantly correlated with FC (r=0,403). HF severity significantly depended from HHD, patient's age group, arrhythmia and MI in the past (r=0,511). CUS was characterized by natural decrease of multiple statistical and spectral indices of heart rate variability (HRV), increase of sympathetic and decrease of parasympathetic control especially for stenocardia and postinfarction patients. Genotype TT carriage associated with increased hazard of CHR course complication. CUS more often had left ventricle (LV) wall thickening and concentric LV hypertrophy (LVH) for 46% in comparison with 30.2% in control group. CHD patients with genotype TT carriage had dilated LV cavity and eccentrics type of LVH. LV systolic function in genotype TT patients changed for the worse if MI was in the past. Diastolic dysfunction was typical for genotype CC patients. Genotype TT of gene PDE4D was a genetic factor that associated with decreased function responsible for endogenous mobilization of CD34(+)CD31(+) cells and these cells hypoactivity in ischemic myocardial region in CUS with CHD and radiation dose less than 50 cGy. In CUS with genotype TT the decreased level of CD34(+)CD31(+) endothelial progenitor cells can be a factor that worsen the course of the disease.
