Purpose - improvement and implementation of early diagnosis and treatment of endocrine diseases in individuals with increased body weight and obesity.Methods: Biochemical, immunoferment, statistics. Equipment: scales, blood pressure cuff, a spectrophotometer, Immunoassay Analyzer, echocardiography. We analyzed the incidence of major endocrine pathologies, which have been found in patients with an increased weight on applications for scientific advice clinic SI "Institute of Endocrinology and Metabolism. VP Komissarenko NAMS of Ukraine". Revealed the following nosology: diabetes mellitus (DM) type 2 - 54.2%, breach of tolerance to carbohydrates - 3.8%, hypothyroidism - 26.8%, hypothalamic syndrome - 14.2%, hypercortisolism - 1%. Among the surveyed children and adolescents - 12% of children were overweight. Overweight was detected in 26.0% of patients with endocrine disorders, 74% - obesity. Obesity level I found in 31.0% of patients, obesity II degree y 56,0%, III degree - 13%. For patients with type 2 diabetes, it found that weight gain is deteriorating carbohydrate metabolism, decreased sensitivity to insulin and increases insulin resistance. Violation of purine metabolism found in 25% of patients with type 1 and 59% of patients with type 2 diabetes. In patients with type 2 diabetes on the background of violations of carbohydrate and lipid metabolism is marked endothelial dysfunction, which constitutes a violation of compensatory possibilities of regional blood flow (according flowdependent vasodilatation of the brachial artery) and high frequency (87.5%) atherosclerotic lesions of cerebral arteries. In patients with type 2 diabetes with increased weight revealed left ventricular diastolic dysfunction in the form of a violation of the relaxation of the left ventricle with preserved systolic function in the background disorders of blood lipid spectrum, namely hypertriglyceridemia. In children and adolescents identified the associative relationship of leptin with systolic and diastolic blood pressure, fasting glucose, insulin and dyslipidemia mainly at visceroabdominal type of obesity. This suggests hyperleptinemia influence on the formation and progression of insulin resistance syndrome and related diseases, further confirms the significantly higher levels of leptin in full symptom of metabolic syndrome.
