Object: the Chornobyl cleanup workers 1986-1987, evacuees, inhabitants of contaminated areas. Subject of study - nontumor morbidity, disability, mortality. Objective: To describe and evaluate post-accident changes in health status of the Chornobyl disaster survivors: nontumor morbidity, disability, mortality. To justify and develop technical approaches to losses of health metrics based on modeling. Methods: epidemiological, mathematical-and-statistical, modeling. Based on the descriptive analysis of the results of long-term cohort epidemiological study covering the period 1988-2015, we described and evaluated post-accident changes in nontumor morbidity, disability and mortality of the Chornobyl cleanup workers 1986-1987 (cohort of 65,345 males), evacuees from 30-km zone aged 18-60 (mixed-gender cohort - 42,982 people), inhabitants of contaminated areas aged 0-60 at the time of the accident (mixed-gender cohort - 98,902 people). Early, remote and late effects depending on time since the accident were determined in post-accident changes in nontumor morbidity among cleanup workers and evacuees. Early effects are characterized by an increased risk of mental and nervous disorders, hypertension, they occur during the first six years; remote effects are characterized by rise in rate of chronic somatic diseases and occur mainly 12-21 years after the accident. Within 22-29 post-accident years, we revealed a gradual decline in the rate of nontumor morbidity against the background of significant increase in nontumor disability and mortality. Rate of disability among cleanup workers was 250 ‰, among persons aged 40-60 at the time of the accident - 325 ‰; and among evacuees - 281 ‰. In the structure of disability, the percentage of circulatory diseases among cleanup workers was 63 % and that among evacuees - 58 %. Death rate from nontumor diseases among cleanup workers increased from 1.5 ‰ (1988) to 17.5 ‰ (2013-2015), among older people - up to 25 ‰; that among evacuees - from 6 ‰ (1988-1992) to 20.6 ‰ (2013-2015). In the structure of causes of death, circulatory diseases amounted to 82.5 % among cleanup workers and 69 % among evacuees. The highest rates for some groups of nontumor (circulatory, respiratory and digestive) diseases among cleanup workers were found in the range of external doses 0.05 to 0.75 Gy. Significant changes were observed in the state status of inhabitants of contaminated areas. Higher rate of nontumor diseases was established within the first 16 post-accident years, especially in the subcohort of people aged under 18 and 40-60 at the time of the accident. In the late post-accident period (22-29 years since the accident), nontumor disability and death rates increased dramatically among people aged 18-39 and 40-60 at the time of the accident. Circulatory, respiratory and digestive diseases, diseases of the nervous system made a major contribution to the structure of morbidity, disability and mortality. During 2013-2015, rate of nontumor disability was 8.1 ‰ among inhabitants of contaminated areas aged 18-39 at the time of the accident and 4.3 ‰ among those aged 40-60. Their death rate was 70.0 ‰ and 50.0 ‰, respectively. We theoretically justified and developed a system of metrics of health changes among the Chornobyl disaster survivors due to nontumor diseases, especially circulatory ones. Implementation: 10 scientific works were published scientific works, newsletter, information-and-analytical reports - 2 (Ministry of Social Policy and Ministry of Health of Ukraine). Fields of application: radiation medicine, radiobiology, health protection.
