Purpose of work: to develop a system of measures to preserve and restore reproductive function in women with uterine leiomyoma in the background of inflammatory diseases of the genitalia based on the study of molecular-genetic mechanisms of development, clinical manifestations, the state of the immune and hormonal homeostasis, morpho-functional state of the target organs and immunohistochemical markers of the development of pathological process. Methods of research: clinical, genetic, echographic, microbiological, virological, immunological, endocrinological, morphological, immunohistochemical. In women of reproductive age with uterine leiomyoma with inflammation of genital found: multiple (69.2%) and submucous nodes (25%), endometrial hyperplasia (40%), pathology of the uterus (62.9%), breast ( 84.2%), thyroid (58.3%), infertility (48.6%), reproductive losses (23.8%), an early manifestation of the disease, rapid growth of nodes, the need for surgical intervention in the active reproductive age (in 5 years earlier). It was found that the development of heavy and clinical manifestations of uterine leiomyoma associated with polymorphisms of ESR1 gene in the absence of regulation of gene disorders PGRі with preservation of reproductive function. The presence of inflammation of genitals and related gene's polymorphism of P 53 leads to disruption of apoptosis and aggravation of pathological changes. Detected of the immune and hormonal homeostasis in the form of an early peak of FSH and a significant increase progesterone level in the middle of the menstrual cycle, increasing the concentration of Ig G, A and the M, both in systemic and local levels, more expressed by the presence of concomitant inflammatory process. In women with uterine leiomyoma combination with genital inflammation process dominated leiomyoma simple type (54.3%), and in patients without a concomitant inflammatory process - cell (73.3%). Chronic endometritis was detected in 70.8% of women. Determination of TNF-? expression showed that it's decrease with inflammatory processes in the II phase of the cycle and the expression of the protein p 53 depended on the proliferative potential on myoma nodes, indicating about the suppression of apoptosis under the influence of progesterone in the presence of inflammation in the pelvic organs and contributed development of uterine leiomyoma. We developed an algorithm of diagnostic and therapeutic measures aimed at restoring the reproductive health of women with uterine leiomyoma with chronic inflammatory diseases of the genital, which is based on an individual phased approach to the choice of tactics of management of patients, depending on the reproductive intentions, size and location myoma nodes and related pathology. This led to an improvement in the clinical course of the disease, normalization of laboratory and instrumental examinations in 66.4% of cases, among which the pregnancy occurred in 82% of patients.
