The object of study - 350 women aged 18 to 35 years. The base group - 300 women with metabolic syndrome (MS), the control group - 50 healthy women of similar age. Objective: To establish a system of prevention and correction of reproductive health in women with metabolic syndrome (MS) through the development of forecasting algorithms, based on the relationships between the state of pituitary-ovarian, pituitary-adrenal system, especially the carbohydrate metabolism and autokrynno-paracrine system adipose tissue. Methods: Epidemiological, anthropometric, medical, statistical, ultrasonic, imunofermetnyy. Measuring means: medical weight for static weighing "Health-2" analyzer "MSR-1000", the ultrasound device Acuson X300 Siemens, automatic biochemical analyzer ADVIA 1800, Siemens, tonometer YADM-OP. The study reproductive health of women with metabolic syndrome showed that they are characterized by clinical signs of relative estrogen dominantosti, the main of which is the increase of fat in the abdominal area, fibrocystic disease of the breast, swelling and fluid retention. Investigation age relationship between the disorders of the reproductive system found that young women of childbearing age increased frequency characteristic oligomenorrhea, various forms of polycystic ovarian active reproductive age, infertility, dishormonal breast disease, amenorrhea, uterine leiomyoma. Features of functioning pituitary-ovarian system in women with MS are relative to background hyperestrogenia hipoprohesteronemiyi and increasing concentrations of androstenedione, total and free testosterone concentration amid falling SSZH, accompanied by a decrease in the fertility index and the increasing concentration of AMH. Established that ovarian AMH synthesis depends on the concentration of total testosterone in the background as hyperplasia of adipose tissue, and without obesity. The tendency to lower concentrations of LH and FSH in women 26-35 years of MS, which indicates the existence of age-forming pathogenic mechanisms mentioned syndrome. Research operation autokrynno-paracrine system adipose tissue showed that 85.7% of women with MS found hyperinsulinemia and dysadypokinemiya :, reduce the concentration of adiponectin and leptin concentrations increase. Increased intra-abdominal fat sized according to ultrasound scans more than 6.0 cm increased the risk of benign breast pathology 2.7 times and increase the percentage of fat according bioimpedansnoho analysis of more than 28% - in 2,8 times, which can be used as prognostic markers of hormone hyperproliferative diseases. Applying a differentiated set of therapeutic and preventive care for women with metabolic syndrome by reducing adipose tissue hyperplasia aimed at normalization of hormonal parameters, restore relationships functional state of adipose tissue, hypothalamic-pituitary-ovarian and hypothalamic-pituitary-adrenal system in patients. For the first time showed a strong effect on adipose tissue dysfunction changing nature of relationships pituitary-ovarian-adrenal and pituitary hormones in women of reproductive age with MS. For the first time based on correlation and regression analysis endocrinological parameters The influence of hormones of adipose tissue on the state of women's reproductive health. This study showed that in conditions of metabolic syndrome as the main link in the pathogenesis of hyperproliferative processes hormone dysfunction should be considered fat, and increasing concentrations of leptin and insulin serve as markers for early diagnosis of hyperplastic above processes. The results of work will be implemented in hospitals gynecological and endocrinological profile. The algorithm of practical advice for women with MS in different age periods. The work is protectable.
