In the last years the incidence of GI cancers are increasing rapidly. The median survival rate of patients without TGF-В expression was significantly lower than that of patients with positive expression. Like to TGF-В, loss of CD44 expression indicate worse prognosis. Patients with second primary esophageal or gastric cancers have significant worse survival compared with routine primary upper digestive tract cancer. Extended gastrectomy with thoracotomy is the better choice of surgical treatment of gastric cardia cancer compared with standart abdominal gastrectomy. Hipertermic intraperitoneal chemotherapy is indicated for patients with metastatic abdominal cancers. In case locally advanced gastric cardia cancer there are high rate of lymph dissemination into thoracic cavity. Selective mediastinal lymphadenectomy should be considered for locally advanced gastric cardia tumours. Therefore identification of possible ways of lymphatic metastases may help to determine the suitable extent of lymphadenectomy.
