Objective - to investigate the mechanisms of the reorganization of erebral hemodynamics and the structural and functional state of the brain after endovascular operations to determine the effect of recanalization of cerebral vassels on the clinical state of patients with cerebral circulation disturbances according to the ischemic type during different periods of the disease, to develop indications for the use of endovascular operations in such patients with purpose of increasing the effectiveness of their treatment. Methods of research - standard questioning, neurological status, cerebral angiography, computer, magnetic resonance imaging, perfusion multispiral computed tomography of the brain, ultrasound duplex vascular scanning, electrocardiography, laboratory indicators. It has been established that the mechanisms for the reorganization of cerebral hemodynamics and the structural and functional state of the brain before and after endovascular operations are ambiguous. Critical to the effective endovascular treatment of these patients is the definition of the state of collateral circulation, the features of the structure of the arteries and the degree of compensation for hemodynamics. The recovery of cerebral hemodynamics after endovascular interventions passed slowly with the development of the syndrome of cerebral hyperperfusion in cases of absence of collateral compensation of cerebral blood flow, presence of critical stenoses of one or more arteries, occlusion or stenosis of other main arteries. Based on the results of the research, a new concept of the endovascular tactics of treatment of patients with ADCC in ischemic type is proposed, which provides for expansion of the "therapeutic window" due to evaluation of the collateral circulation of the brain, determining the most effective way of performing cerebral revascularization and use not only "bridg?" concept in the treatment of ischemic stroke, but also the use of mechanical and vacuum thrombextraction as an independent and effective method of recovery the cerebral artery patency. New knowledge about the patterns of restructuring the structures and functions of the brain and their relationship, which are obtained are associated with high-tech endovascular interventions. The technology of preventive and therapeutic endovascular interventions in ischemic disorders of cerebral circulation in different periods of the disease and the technology of prevention of distal embolism during the execution of mechanical thrombextraction in acute ischemic disorders of cerebral circulation were developed. The method of increasing the effectiveness of endovascular treatment of patients with ischemic impairment of cerebral circulation was developed (patent of Ukraine No. 115983 of 10.05.2017). Efficiency: The effectiveness of treatment of patients with ischemic brain lesions improves the introduction of the results of this scientific research into medical practice and makes it possible to carry out secondary prevention of stroke, which reduces disability and mortality among these patients, reduces the material costs of their treatment, rehabilitation and social adaptation.
