Methods - questionnaires, clinical, functional, immunological, statistical. The basic clinical and functional features of the severe course of bronchial asthma (ВА) have been identified, which allowed to distinguish the following separate phenotypes of the disease at the specialized medical care level: allergic and non-allergic asthma, late onset asthma, asthma with fixed bronchial obstruction, asthma chronic obstructive pulmonary disease (COPD) overlap, asthma with comorbidities. It has been established that patients with severe BA do not receive proper basic therapy - 43,8% of patients with late-onset asthma do not receive a combination of inhaled corticosteroids and long-acting beta-2 agonists at appropriate doses, and 71,4% of asthma patients with fixed bronchial obstruction and with asthma COPD overlap does not receive tiotropium bromide, which indicates the necessity to isolate separate phenotypes of the disease and the appointment of pathogenetically substantiated treatment. It was determined that the basis of secondary prophylaxis of severe asthma consists of the early detection of asthma phenotypes with the timely appointment of adequate therapy and high patient adherence to the treatment. The new technology for the diagnosis of severe bronchial asthma phenotypes at the specialized medical care level has been developed, which allows to identify an additional phenotype of asthma with fixed bronchoconstriction in 14,4 % of patients in patients with a possible combination of BA and COPD and to improve the accuracy of the diagnosis: allergic asthma - by 10,0 % (with allergic rhinitis - by 8,8 %, polyposis ethmoiditis - by 5,6 %), non-allergic asthma - by 10,0 %, asthma COPD overlap - by 14,4 %, asthma with comorbidities - cardiovascular disease systems - by 23,2 %, of obstructive sleep apnoe-hypopnea syndrome - by 10,6 %.
