Methods - clinical, biochemical and functional. Results of the research. It was established that in patients with DN progressive increase of fibroblast growth factor 23 (FGF - 23) level in blood plasma takes place depending on the stage of disease. Highest levels of FGF - 23 were found on the late stages of disease. As compared to controls and early stages of DN on the late stages of disease mineral disturbances were characterized by hyperphosphatemia. It is well-proven that in patients with DN increased levels of FGF23 associated with duration and compensation of diabetes mellitus. Between the state of myocardium, cardiac hemodynamics and activity of the system FGF23/phosphoric-calcium metabolism, especially FGF23, there was found close association that is probably mediated not only through the rennin-angiotensin-aldosterone system. In addition, close correlations were found between the concentration of phosphorus and myocardium thickness of back wall of the left ventricle (LV) and interventricular septum, as well as volume indexes of LV, and also strong negative correlations with discharge faction of LV. Base therapy resulted in the FGF23 levels decline in all investigated groups: for patients with the initial stages of disease it was found more substantial decline of FGF23 when compared to patients with proteinuria and hyperphosphatemia. Base therapy lead to the improvement of renal functional state and lipid metabolism while reliable changes of mineral metabolism in patients with hyperphosphatemia were not found. Complex therapy with nicotinic acid use resulted in blood FGF23 level decline in patients with DN.
