The purpose of the study: to determine the value of melatonin (Mt) and other biogenic amines in the pathogenesis of hormonal and metabolic disorders in patients with pituitary adenoma and based on the results to justify the path to their correction. Methods of research: clinical, biochemical, immuno-enzyme, instrumental, statistical.The degree of severity of carbohydrate metabolism disturbances in patients with somatotropic (ST) and somatomamptoporin (SMT) is associated with age and overall duration of the disease, and among the possible mechanisms of hyperinsulinism development in patients with acromegaly, one can consider the stimulating effect of hypersomatotropinemia and high concentration of insulin-like growth factor-1 (IGF -1) directly to the pancreatic beta -cell, reduced tissue sensitivity to insulin-mediated glucose assimilation and impaired clearance of insulin. Infringement of lipid metabolism was established in 77,4% of patients. The degree of severity of these disorders is influenced by such factors as the age of the patient, the central type of distribution of fat, the level of somatotropinemia and the state of glucose homeostasis associated with it, as well as the inversion of the daily rhythm melatonin (Mt), which was found in 62.5% of the surveyed, whose value is no less significant than the level of IGF-1 and somatotropic hormone (STH). The presence of carbohydrate metabolism disorders only in patients with pituitary adenomas producing somatotropic hormone (SMT, ST), and a significantly higher percentage of detection of dyslipidemia in these patients suggests that the state of hypermototropinemia plays a decisive role in the formation of violations of carbohydrate and lipid metabolism. In the study of pituitary-epiphyseal relationships, it has been established that in patients with acromegaly, as well as in healthy persons of the corresponding age, Mt is a suppressor of gonadotropic pituitary function. At the same time, when analyzing the level of prolactin in the blood from the daily excretion of Mt, reliable data was obtained neither in patients with acromegaly, nor in patients with prolactinoma. Disturbance of the circadian rhythm of secretion Mt is a determining factor in the cardiovascular risk associated with ST and SMT, namely, arterial hypertension and dyslipidemia. Normalization of the ratio of day and night melatoninemia due to the appointment of a synthetic analogue Mt leads to a decrease in blood pressure, correction of dyslipidemia and can be recommended for use as an additional pathogenetic therapy in patients with hormonal-active pituitary adenoma.
