Purpose: on the basis of conducting of retrospective, genetic and clinical examination of patients with follicular neoplasia of the thyroid gland optimize the tactics of surgical treatment of this pathology. Methods of investigation - general-clinical, immuno-enzyme, sonographic, pathomorphological and immunohistochemical methods of research of thyroid tissue, methods of formal genetics, statistical. An immunohistochemical study of surgical material (removed thyroid tissue) was performed on 50 patients with thyroid follicular neoplasia. A high level of affinity for benign adenomas of thyroid gland and FBS in terms of proliferation (Ki-67), disorders of apoptosis mechanisms (p53 and BCL 2), cell differentiation and functioning (NIS and E-cadherin), angiogenesis (MMP9 and CD34). Solid and fetal-embryonic thyroid follicular adenomas had properties that are more characteristic of the malignant process, as opposed to normal (normofocylic) adenomas. It is likely that these types of follicular neoplasia have the highest potential for malignancy and subsequent metastasis. The analysis of the family accumulation of diseases and hereditary predisposition to TBC was carried out by means of a genetic survey of 45 patients with FGD, 120 patients with TB and 178 patients with TB. The obtained data testify to the polygenic inheritance of FRCH, and the detected frequencies of FGD patients among relatives of the first and second degree of kinship indicate the familial nature of the accumulation of the disease. The molecular genetic study of P12A and H449H polymorphisms of the PPAR? gene did not detect mutations in patients with PRHC (control group). A new tactic for patient management was developed and tested with the combined application of TI-RADS / TBSRTC. Combined use of diagnostic systems (TIRADS and TBSRTC) provided more accurate data for individual prediction of results. The positive value at the simultaneous application of the ultrasound and cytological systems had a higher sensitivity (93.2%) than each of the tests separately. Specificity increased to 96% versus 88 and 93% for each test, respectively, the prognostic value of testing for no malignancy increased to 97.7%. The combined use of the two systems provided a more accurate individual prognosis: almost 90% of the positive result for the cancer of the thyroid tumor was confirmed in the final pathomorphological study. The developed and tested technology of high-frequency welding of tissues during surgical interventions on thyroid gland is highly effective and safe due to effective hemostasis, reducing the number of complications, duration of operation and anesthetic provision of the patient. We conducted a study of Se's level of hair in women of child-bearing age 25-55 years old and teenage girls living not less than 10 years in Dnipro, Odessa, Sumy and Kharkiv, and Kharkiv and Zaporozhye regions. The data of the conducted research allow to draw a preliminary conclusion about the presence of expressive selenoid deficiency in Dnipropetrovsk, Odesa, Sumy and, especially, the Kharkiv regions.
