The thesis is devoted to chronic pancreatitis treatment improvement through diagnosis and surgical correction of intraductal pancreatic hypertension (IPH). As a result of clinical research it is revealed that recurrent pain syndrome, index mass body decrease below 16,3, alfa-amylase level increase above 1024 un., widening of pancreatic ducts to 4,7 mm., are the predictors of intraductal pancreatic hypertension in patients with chronic pancreatitis. Use of SF-36 questionnaire, which is recommended by the International center of investigation of quality of life and VAS (visual analog scale) for pain syndrome assessment along with clinical and laboratory-instrumental methods of intraductal pancreatic hypertension diagnosis permits to achieve more precise election of methods of treatment of patients with chronic pancreatitis. Oppression of pancreatic exocrine secretion and recuperation of pancreas evacuation activity increase treatment effectivity to 12,8%. Fray - Izbicky pancreatojejunostomy allows it to decrease the number of relapses of intraductal pancreatic hypertension to 11,1%. Endoscopic papilosphincterotomy with nasobiliar drainage and endoscopic pancreatic duct dilatation are the most suitable treatment in obstructive cronic pancretitis.
