Aim - improving the efficiency of treatment of children with asthma, on the basis of new scientific evidence on the association of polymorphic variants of genes and the establishment of candidate genes that determine individual response to drugs are the basic treatment, the formation of pheno-genotypes of bronchial asthma in children and the development of the algorithm of differentiated selection schemes base therapy of pheno-genotypic differences. Methods - clinical, questionnaires, functional, immunological, molecular genetic, mathematics, statistics.The leading genotypes that modify the risk of uncontrolled asthma and increase this by 2-3 times have been established: genotypes AT2R1 CC/AC, eNOS T-786C CC and NAT C41T TT; genes associations as AT2R1 AC with eNOS T-786C CC, AT2R1CC with NAT C41T TT; gene- factor associations as asthma with level of total IgE 400 IU/L and more and genotype AT2R1 AC/CC and/or eNOS T-786C CC. A technology to determine the risk of uncontrolled asthma in children had been developed by the functional change index and the Cerdo index which at high risk increases the amount of basic therapy and accelerates asthma control in 70.0 % of patients, who ineffectively had been treated previously, and also reduces the frequency of exacerbations of asthma by 30,0 %. An algorithm for predicting the risk of adverse asthma in children had been developed by calculating the sum of prognostic thresholds for the most informative clinical and anamnestic markers, confirms the risk of moderate or severe asthms with a confidence of 95.0 % and to disprove this risk with a confidence of 80.0 %. A method for the treatment of persistent bronchial asthma in children with IgE-phenotypes and different genotypic features for the AT2R1 gene had been developed, that increases asthma control by 3.4 times.
