As a result of performed clinical and morphological comparison of gliomas of different histological structures, it has been established that the typical phenomenon for malignant tumors of the brain is the defeat of several parts of the brain, including the involvement of median structures. In the structure of detected tumors in older people, glioblastoma was most often determined (GB, 54.2%). Structural signs of hypoxia in the zone of invasion of NEP were detected in 83% of observations, with the most expressive degree of hypoxia inherent in high-grade tumors (G III-IV), GB and anaplastic ependymomas (67% and 15% respectively). In patients with GB, the indicators of proliferative activity averaged 26% + 1.2%. In patients of the older age group, apoptotic activity rates are higher than in the comparison group of 35.8 to 54.2% in the main group of 61.3 to 37.9%. The ultrastructural features of malignant neoplasms consist of a change in the structure of pericarpular spaces, which proves the presence of a violation of the morphology of hematoencephalic barrier, but direct correlation with the age of patients is not established. The results of morphometry were statistically significant in reducing the area occupied by mitochondrial profiles in the cytoplasm of tumor cells, size of mitochondria and / or changes in their fine ultrastructure, which in our opinion correlates with an increase in anaplasia and metabolic changes in macroglial tumor cells. According to the data of the inter-group comparative analysis, there was a statistically significant difference between the anaplastic astrocytomas (AA) and GB: the percentage of VEGF-immunopositive cells in GB was statistically significantly higher than in the AA in 1.3 times (90.5 (88.4, 95.9, respectively) )% and 70.0 (63.6; 73.5)%; U-Mann-Whitney criteria; p = 0.0000). In patients with GB and AA, suppression of the production of active forms of oxygen by monocytes of peripheral blood was established. The level of spontaneous production of superoxide anion radical has been reduced by 31% compared with the control. A lower level of Hoechst33342 + apoptotic cells among peripheral blood lymphocytes was found in patients with gliomas of III and IV degree of malignancy compared with gliomas of the I-II degree. In tumors, the content of Hoechst 33342+ apoptotic cells decreases with an increase in the degree of malignancy by glioma. The scheme of integral morphological indices and a morphogenesis subdivision of the cerebral NEP, which provides a personified approach to treatment, is developed, depending on the general condition of the patient, metabolic features and tumor localization.
