Scope of application: Cardiological and cardio-surgical centers, hospitals of cardiology, polyclinics. Object of research: adolescents and adults with obstructive congenital defects of the right heart. Objective: To increase the efficiency of diagnosis, medical treatment and to substantiate the indications for surgical interventions in adolescents and adults with obstructive congenital defects of the right heart. Research methods: Anamnesis, electrocardiography, ultrasound examination of the heart and vessels, radiography, computed tomography, magnetic resonance imaging, loading tests, angiographic and arrhythmologic invasive studies, psychodiagnostic methods, statistical and mathematical analysis of the obtained data. The development of cardiosurgery in the world and in Ukraine has made it possible to improve the survival of children with congenital heart disease (AVC). The frequency of airborne infants in children ranges from 4 to 10 per 1000 live births. 30-40 years ago in the developed countries of the world the question arose about the organization of providing surgical care first to children of early age, and then to the newborns. Gradually, the emphasis on improving the assistance to patients from the Air Force has shifted from the pediatric service to the formation of a cardiological care structure for adults with this pathology of the heart. In Europe and the United States, up to the age of 18 live around 85% of patients born with an airborne disease. Every year in Ukraine, about 4,000 children from the Air Force are born. The dynamics of the prevalence rate and incidence of airborne diseases in children is noted, which will be reflected in similar figures from the air force of the adult population. The prevalence of adult patients with airborne illness is not clearly known. The estimated prevalence of Air Forces in adults can range from 4.0 to 6.0 per 1000 adults. Extrapolating the data obtained to the population of Ukraine it can be predicted that the number of adults from the air force in our country ranges from 153000 to 225000 people, but there is no reliable value for this indicator. Adolescents and adults from the Air Force are not only diagnosing, but also forecasting remote results of these abnormalities. The total number of patients with obstructive congenital defects of the right heart, aged 18 years, consists of patients operated on the Air Force, as well as those who do not require prompt correction of existing defects. In the structure, patients with defects will be operated, and further prolonged survival is possible without additional surgical intervention or other invasive procedures. In such patients, the anomalies, a concomitant problems, such as infective endocarditis, thromboembolism, rhythm disturbances, etc., will come to the fore. This reflects a great focus on identifying and early diagnosis of complications, while for the patient the indicators such as quality of life, psychosocial factors are becoming more significant: professional orientation and choice of specialty, the ability to plan pregnancy and the tactics of child support, sports, etc. Almost all Air Forces can perform surgical corrective interventions for the purpose of both normalization and adaptation of hemodynamic changes. B as a result of the intervention can be obtained almost completely anatomically restored heart or only surgically modified, which maintains one or another deviation from the norm. However, the issue of justification and indications for repeated surgical intervention in adults with obstructive congenital defects of the right heart organs, the choice of the procedure of surgery, the terms in which surgery should be performed, and who should perform surgical treatment remain unsolved. Developing a strategy for assisting patients over the age of 16 with obstructive congenital defects in the right heart departments will identify factors that reduce the survival of patients with airborne diseases after surgery, worsen quality of life, and affect life expectancy in the long-term observation period, and will allow the introduction of phased and sequential care adults from the Air Force, forming a multidisciplinary team to assist these patients.
