The purpose of the study: based on the study of pathophysiological mechanisms, to develop quantitative indices (instrumental and laboratory) for evaluation of the function of the single ventricular myocardium myocardium to detect early signs of ventricular dysfunction and to develop medical and surgical methods of correction. Methods of research: ultrasound (transthoracic echocardiography), catheterization of the cavity of the heart with angiography, multizirious computer tomography with intravenous administration of contrast substance, magnetic resonance imaging, statistical methods. On the basis of complex multimodal assessment of hemodynamics and ventricular myocardial function in 137 patients at the stage of complete hemodynamic correction using ultrasound, angiographic and radiological methods of research developed within the framework of the GDR, quantitative indicators of functioning of the heart with a single ventricle of different morphological groups were obtained. The study of the anatomical features of a single ventricular myocardium reliably has the advantage of dominant left ventricular morphology compared with right ventricular and mixed with systolic and diastolic function parameters (TAPSE / MAPSE, Tei Index, IVF and single ventricular vial) before and after complete hemodynamic correction. The predictors of unsatisfactory results of surgical surgical interventions (pTCP> 20 mm Hg, PVR> 2.9 OD / m2, CKD ? 14 mm Hg, ESF <45%) were detected. Biochemical markers (galectin-3 and NT-proBNP) of myocardial function correlated with Tei Index for various types of single ventricular morphology and cardiac failure progression. The detection of early signs of systolic and / or diastolic dysfunction of the single ventricular myocardium helps to correct drug therapy and to determine the optimal timing of surgical interventions that affect the quality of life of patients and remote surgical outcomes in patients with a single ventricle of the heart.
