The scientific work investigates the complex influence of systemic inflammatory factors and other biochemical predictors on the development of endothelial and myocardial dysfunction in various forms of coronary artery disease. The peculiarities of disorders of myocardial and endothelial functions in patients with various diseases of the cardiovascular system (coronary artery disease, arterial hypertension, chronic heart failure, atrial fibrillation) were investigated and a comprehensive assessment of these factors for the quality of life of the examined patients was conducted. Clinical and instrumental predictors of the development of various forms of coronary artery disease and possible complications are highlighted. It has been shown that atrial fibrillation acts as a direct pathogenic factor in the progression of myocardial dysfunction in patients with coronary artery disease and hypertension. Pathogenetic factors of progression of myocardial dysfunction in this category of patients have been elucidated. Research conducted during the scientific work allowed us to develop a differentiated approach in the treatment of patients with hypertension, depending on the daily profile of blood pressure; prognostic value of coronary artery disease depending on biochemical markers and state of the renin-angiotensin-aldosterone system. In the course of the research, specific algorithms for selecting effective treatment for these patients were developed, ways to improve the antiarrhythmic efficacy of propafenone in patients with recurrent atrial fibrillation. The results of the work were implemented in a number of medical activities in Ukraine, 134 materials were published according to its materials, 7 patents for utility model were issued, 3 dissertations were defended, 4 dissertations are in the process of implementation.
