Lukyanovaya NAMS of Ukraine» popup.description2  Extragenital pathology in pregnant women at risk of venous thromboembolism is dominated by cardiovascular disease (54.8%), obesity (16.4%) and connective tissue disease (14.5%). Women who underwent venous thromboembolism prevention were characterized by: high risk obstetric history (60.9%), risk of termination of pregnancy (27.9%), fetal distress (18.7%); a significant proportion of operative births: caesarean section (34.8%) and vacuum extraction (8.4%); high incidence of complications of the third period (16.7%), incl. increased blood loss. Perinatal losses amounted to 2.68%, 40.2% of babies were born in asphyxia. The novelty of the work is that due to the conducted researches the influence of various risk factors of venous thromboembolism on the course of pregnancy, maternal and perinatal consequences in pregnant women with extragenital pathology was established. The risk factors for the development of side effects caused by anticoagulant prophylaxis in pregnant women with extragenital pathology were determined, the features of pregnancy and the condition of the fetus were studied depending on the serological indicators of antiphospholipid syndrome and the presence of hereditary thrombophilia. An algorithm for the diagnosis of heparin-induced thrombocytopenia and timely correction of anticoagulant therapy is proposed to prevent the progression of this complication. Treatment-and-prophylactic measures for pregnant women receiving anticoagulant therapy were introduced, which reduced the frequency of low-intensity bleeding by almost three times, and the progression of osteopathic changes fourfold.
