Purpose: To reduce the risk of postoperative complications in patients with multiple organ surgeries by individualizing intensive care tactics. Research methods and equipment: clinical-instrumental, biochemical, clinical, statistical; cardiac monitor Hiaco, ultrasound, electrocardiography. Theoretical and practical results: the program of monitoring of intensive care in cancer patients in the postoperative period is substantiated. New data on the use in the complex of intensive therapy of hemofiltration and its effect on the severity of the condition have been obtained. New hemofiltration techniques and treatment regimens with dobutamine and nitroglycerin have been proposed, which has reduced the risk of cerebral and cardiohemodynamic disorders by 65%. Recommendations on the clinical use of predictors of possible complications have been developed. Novelty: for the first time in Ukraine, a scientifically validated approach for the individualization of intensive care tactics in cancer patients after multiorgan operations has been developed and tested. Optimal algorithms for diagnostics and intensive care in patients with multiorgan operations have been substantiated for the first time. Implementation efficiency: The complex application of the developed algorithms in cancer patients after multiorgan operations, has allowed to reduce postoperative complications from 67.5% to 38.5%, or 1.7 times, and the mortality rate by 1.9 times. The economic benefit is to reduce complications 1.7 times, which reduces the cost of treatment per day from 2500 UAH to 1000 UAH. The developed algorithms allowed to reduce the stay of patients in the clinic from 19.8 ± 3.2 days to 12.4 ± 2.7 days. The economic benefit is to reduce the cost of treatment of the patient for the institution from 11325 UAH to 7092 UAH saving 4233UAH. Field of application: oncology and intensive care.
