Research methods: clinical and statistical, ultrasound exam of the fetus, invasive procedures with cytogenetic analysis of chorionic/placenta bioptate, amniotic fluid, fetal blood by conventional methods. Equipment: ultrasonic diagnostic scanners ACCUVIX V20EX-EXP, ACCUVIX V10LV-EX. Comparative analysis of data on complex prenatal investigations, pregnancy and labor course, and postnatal outcomes in congenital diaphragmatic hernia, gastroschisis and high intestinal atresia was performed. High variability of clinical and anatomical features of these malformations was demonstrated, which results in variability of clinical outcomes, and requires a systematic standardized approach to prenatal examinations. Dynamic changes of prenatal clinical features during pregnancy were demonstrated, which complicates prediction of perinatal outcomes in the second trimester. Mathematically and clinically grounded, term-independent prognostic indices were proposed. The prognostic value of mathematical and clinical anatomical markers has been determined. A standardized algorithm for prenatal examination with a focus on common and specific prognostic markers was scientifically substantiated. Clinical and anatomical classifications of these malformations were developed, which allows to distinguish groups of patients with different prognosis - favorable, doubtful or unfavorable. According to the prognosis for the newborn, an individualized management of pregnancy and delivery, as well as specialized help to neonates is planned. The results of the implementation of prenatal dispensary system and management of patients in a single medical institution were demonstrated, namely the reduction of postoperative mortality in congenital diaphragmatic hernia and gastroschisis.
