The aim of the work is to study the spectrum of genomic changes in groups of patients with Ph-negative MPNs in specific genes - drivers (major mutations) with subsequent identification of other potential genes that may be involved in the development of tumors. Research methods - genomic DNA was obtained from peripheral blood leukocytes, identification of major mutations was performed by polymerase chain reaction, additional genetic variants were studied by complete exomic and target sequencing, performed statistical processing of the obtained data. It was confirmed that patients with a history of radiation are more likely to have a negative status for major driver mutations in the genes JAK2, MPL and CALR than those who became ill spontaneously (25.7% vs. 15.7%, p = 0.046). The mean number of additional mutations in patients with radiation-associated primary myelofibrosis (PMF) was 4.8 per case, while spontaneous - 3.1 (p = 0.024). In the group of patients with PMF with a radiation history and triple-negative mutation status, the most frequently altered genes were EZH2, DNMT3A, TET2 and ATM.
