Object of study. Group of patients with critical congenital heart disease (critical aortic stenosis, critical aortic coarctation, hypoplastic aortic arch, interrupted aortic arch, critical pulmonary artery stenosis, pulmonary artery atresia, transposition of great arteries with intact IVS, total anomalous pulmonary venous return, hypoplastic left heart syndrome). Purpose. to reduce mortality and disability rates for patients with critical congenital heart disease by improving routing, diagnostics and cardiac surgery management in critical patients. Research methods. Blood test, chemistry panel, ECG, echoCG, CT scan, angiography, statistics, analysis of pathomorfological conclusion. The analysis of surgery management of congenital heart disease: critical aortic stenosis, critical coarctation of the aorta, hypoplastic aortic arch, interrupted aortic arch, critical pulmonary stenosis, pulmonary atresia, TGA, HLHS, total anomalous pulmonary venous return. The optimal terms and criteria of prenatal diagnosis of critical congenital heart disease are determined. The main causes and lethal consequences in infants with complex congenital heart disease have been studied. The importance of surgical and endovascular treatment of critical congenital heart disease have been established. Protocols of improved management of infants with critical congenital heart disease (protocol of management in the delivery room, screening protocol of pulse oximetry, protocol of prostaglandins using depending on the clinical condition of the patient, algorithm of medical support of patients with critical congenital heart disease from maternity hospital to cardiac surgery division). Algorithms for diagnostics and methods of intervention in deep-premature infants with persistent PDA have been developed and implemented.
