The aim of the study: is to increase the effectiveness of radiation therapy for head and neck cancer by optimizing topometric preparation using modern imaging techniques. Research methods and equipment: X-ray, morphological, clinical-instrumental, statistical, linear accelerator "Clinac-600C", planning system Eclipse 8.9, X-ray computer tomograph "Toshiba Aquilion 64", simulator "Acuity", magnetic resonance imaging, apparatus for ultrasound examination, brachytherapeutic complex "MultiSource", light microscope, device for monitoring the parameters of the radiation beam of a linear accelerator. Theoretical and practical results: evaluated the sensitivity, specificity and accuracy of computed tomography in the topometric preparation of patients with head and neck cancer. MRI topometry has been shown to reduce radiation exposure by 12.5% in the first stage of EBRT compared to classical CT scans. The method of preoperative course of combined radiation therapy with HDR-brachytherapy by dynamic fractionation has been developed. Novelty: the scientific data on radiation toxicity have been supplemented, in particular, the development of grade 3 radiation reactions is 2 times less when planned according to computed tomography data. It has been proven that radiation therapy using HDR brachytherapy according to electron microscopy increases the level of tumor regression by 15%. Effective implementation: reduction of the duration of treatment and the cost of treatment of radiation complications, which averages UAH 4,350 per patient per course of treatment.
