Purpose: to develop rational approaches to the treatment of patients with typical TP and AF of nonvalvular genesis based on the study of clinical features, structural and functional state of the myocardium, electrophysiological characteristics of arrhythmia, indicators of the intensity of systemic inflammation and activity of the renin-angiotensin system.
325 patients with TP and AF of non-valvular genesis were examined: 285 (87.7%) men and 40 (12.3%) women aged 18-83 years (on average (57.18 ± 0.57) years).
Research methods: clinical examination, ECG, HMEKG, DMAT, transthoracic and transesophageal echocardiogram, RFA CTP, prospective and retrospective observation.
It has been proven that patients with a persistent form of atrial fibrillation-flutter and paroxysms lasting more than 7 days were characterized by a significantly longer history of arrhythmia, a higher frequency of paroxysms, pronounced HF, thyroid gland diseases, as well as significantly smaller amplitude of the F wave on the surface ECG and the amplitude of the A wave on ChSEG than in patients with isolated TP, which is associated with the processes of electrophysiological remodeling in the atria. In these patients, a significant decrease in the effectiveness of CHSEX, more frequent use of EIT and preservation of permanent forms of arrhythmia were revealed. The effectiveness of treatment of typical TP by the method of catheter RFA of KTP was 94%. Within 24 months after a successful procedure, 45% of patients were diagnosed with AF. The main predictor of the appearance of AF after RFA of CTP is the ejection velocity of the AF from the auricle of 40 cm/s or less.
