Research methods: clinical supervision, coronary angiography, cycle ergometry, echocardiography, indicators of a general blood analysis, biochemical indicators, determination of precursor cells endotheliocytes, determination of the level of PCSK9 before and after physical exercise. The purpose of the study is to establish the pathogenetic mechanisms of the progression of the atherosclerotic process in patients who suffered a myocardial infarction against the background of post-traumatic stress syndrome as a result of military operations in the conditions of the Covid-19 pandemic according to the clinical course, recovery of myocardial kinesis , the ability to restore the function of the endothelium to produce endothelial progenitor cells (ECP) at the height of the stress test, the level of PCSK9 in plasma. 36 patients with cardiovascular diseases and Covid-19 and 88 patients with ACS were examined in the dynamics of annual follow-up. In the structure of cardiac pathology and comorbidities, acute infection with SARS-COV-2 occurred against the background of hypertension in 82.9% of patients, AF in 25.7%, HF IIA stage in 57.1%, diabetes in 22.9%, and in addition to hypertension, these conditions were significantly more frequent in the group of older/elderly patients and were accompanied by higher levels of CRP (three times higher than normal) and D-dimer. A complex of rehabilitation measures (including physical training) contributed to an increase in TFN from 69 W to 85 W, an increase in LVEF from 50.5% to 53.5% already after three months, mainly at the expense of patients undergoing a course of physical training. In 90% of patients there was a decrease in the level of PCSK9 after FN, and in half - an increase in the number of KPE, which indicated the reserve possibilities of recovery after MI.
