Specific features of the severity condition of patients with acute IS caused by occlusion of large cerebral artery (CA) from hospitalization and after endovascular reperfusion studied. It was established that MSCT is method of diagnosing early ischemic changes in acute IS. Non-contrast MSCT allows to determine not only early signs of IS, but also early signs of CA occlusion. Determined factors of the efficiency of endovascular reperfusion: the volume of the hypoperfusion zone, the volume of the brain damage zone according to the initial MSCT data, the quality of the performed reperfusion, patient's age, concomitant pathology, time since the onset of the stroke, collateral compensation in the region of the thrombosed artery, complications during operation. With direct thromboaspiration, first pass efficiency factors are: cardioembolic stroke, localization of occlusion in the middle cerebral artery, favorable anatomy of the internal CA siphon, and pronounced collaterals. During stent-retriever recanalization, first pass efficiency factors are: cardioembolic type of stroke, localization of occlusion in the vertebrobasilar zone, pronounced collateral compensation of the zone of the thrombosed artery. In the combined techniques group, first pass efficiency factors are: localization of occlusion in the middle cerebral artery, favorable anatomy of the internal CA siphon, pronounced collaterals, time interval of up to 6 hours from the occurrence of stroke. Complications during endovascular reperfusion in patients with IS have been determined. The results of acute CA occlusion treatment 90 days after interventions were analyzed. New approaches have been introduced into practice and practical recommendations have been developed to increase the effectiveness of endovascular recanalization methods in patients with IS.
