On the basis of the determined hormonal, immune, bacteriological, psychological, histological, instrumental-diagnostic parameters, the pathogenetic mechanisms of the development of premature birth were specified. It has been established that inflammatory reactions in the placenta develop in the dynamics of the placenta's evolution, and as the gestation period increases, the signs of inflammatory reactions increase in quantitative characteristics and spread to all structures of the placenta. That is, it has been proven that the prolongation of pregnancy to increase the gestational age of the fetus, the term of gestation (in the absence of dangerous conditions in the mother and the fetus) is reasonably safe and contributes to increasing the survival of infants and reducing their disability. A set of measures (treatment, childbirth tactics) has been developed for each period of pregnancy. The study of the mechanisms of the implementation of infectious agents in the mother-placenta-fetus system established that the bacteriological-virological associations that lead to PRPO develop from the period of pre-gravid remodeling of the endometrium, their implementation is associated with the genetic prerequisites of the mother and fetus (in particular, factors of innate immunity TLR-9) and immune responses occurring on the mucous membranes, in particular the cervical canal (the lysozyme system and immunoglobulins of various classes).
