The goal of the NDR was to optimize the complex treatment of patients based on the study of morpho-functional features for local and systemic routes of administration of a chemotherapeutic drug; increasing the safety of long-term chemotherapy by developing an alternative vascular access, which consists in the formation of an arteriovenous fistula on the upper extremity. To achieve the goal, it was necessary to solve the following tasks: to improve and test an experimental model for serial intravesical administration of drugs in rats. To conduct a comparative assessment of caridotoxicity of doxorubicin for systemic and local application. To evaluate the morphological changes of the myocardium of rats after serial systemic and local administration of doxorubicin. To study the clinical and morphological features of damage to peripheral veins after the administration of cytostatics. To develop and implement the indications and methods of creating a subcutaneous arteriovenous fistula for the purpose of antitumor therapy. To determine the effect of arteriovenous fistula on the state of central hemodynamics of oncological patients. To analyze the results of operation of arteriovenous fistulas in cancer patients. The conducted clinical and experimental studies substantiate the expediency and safety of serial intravesical use of doxorubicin in the complex treatment of bladder cancer.
