The choice of the technique of coronary stenting should be determined by the features of the lesion not only of the trunk, but also of the main and side branches. In order to achieve optimal immediate and long-term results, a necessary condition is compliance with the intervention methodology, especially in patients who require the initial use of a two-stent technique. It should be noted that the use of a single-stent technique may lead to the need to switch to a two-stent manipulation. The easiest way is to switch to the TAR and culotte method. Intravascular imaging is a prerequisite for performing stenting of the LCA trunk, as it allows for the most accurate selection of the necessary equipment to reduce the risk of restenosis in the stented segment in the distant period. One of the main reasons for the recurrence of angina pectoris is the progression of atherosclerosis, which is due to low patient compliance with drug therapy. LCA stem stenting requires a highly qualified operator and requires discussion by a team of specialists. Coronary bypass surgery is an effective way of treating patients with hemodynamically significant damage to the trunk of the coronary artery (69.2% in the remote period). Recurrence of angina pectoris was noted by patients in 26.7% of cases. The most frequent causes of deterioration in both studied subgroups were the progression of coronary atherosclerosis in new locations, degenerative changes of shunts, and a combination of these two processes. In solving the problem of angina recurrence in patients with a history of coronary bypass surgery, the surgeon who performed the operative treatment and the cardiologist should participate in addition to the interventional cardiologist.
