The purpose of the proceedings: To identify specific regulatory microRNAs associated with tumor sensitivity to neoadjuvant chemotherapy and to develop an algorithm for individualized treatment of patients with locally advanced oral and oropharyngeal cancer. Research methods - clinical, pathomorphological, immunohistochemical, endoscopic, radiologic, polymerase chain reaction analysis. The association of microRNA-10b and -155 expression levels with lymph node metastasis, tumor differentiation grade, and Ki-67 levels did not differ between the groups of p16INK4a-positive and p16INK4a-negative tumors (p > 0.05), indicating the possibility of using these microRNAs as prognostic markers for oral squamous cell carcinoma independent of p16INK4a status. It was proved that the levels of microRNA-21 and -155 were positively correlated with TNM T (r=0.68 and r=0.75, respectively) (p<0.05) and TNM N (r=0.78 and r=0.71, respectively) (p<0.05). The expression of microRNA-375 was negatively correlated with TNM T (r=-0.67) (p<0.05), and its level was 2.1 times higher in saliva of patients with N1-2 than in saliva of patients with N0 (r=-0.59). Analysis of microRNA-21, microRNA-155, and microRNA-375 in relation to tumor response to neoadjuvant chemotherapy showed that only microRNA-21 (p=0.004) and microRNA-375 (p=0.01) were associated with response to therapy. It was found that the levels of microRNA-21, microRNA-155, and microRNA-375 in saliva in 85% of patients diagnosed with OSCC and OPSCC with tumors resistant to NACT were above 10.1, 9.8, and below 1.1 с.u., respectively, while 85% of patients who responded to NACT had lower levels of microRNA-21 (<9.2 c.u.), -155 (<9.4 c.u.), and higher levels of microRNA-375 (>2.1 c.u.). The research resulted in the development of an algorithm for individualized treatment of patients with locally advanced OSCC and OPSCC based on the identified specific regulatory microRNAs.
