The ApoB/ApoA1 index in patients with severe coronavirus disease was critically high, which indicated possible further complications from the cardiovascular system and kidneys and may be evidence of the formation of type 2 cardiorenal syndrome. The level of IRS-1 in the blood of patients with severe COVID-19 was significantly higher than in healthy people, the DM group and the COVID-19 groups with and without comorbidities. It is likely that the causes of severe COVID-19 were high BMI and, especially, extremely high blood glucose levels. The progression of cardiorenal syndrome СRS in patients with type 1 and type 2 DM occurs by different mechanisms. Patients with type 2 DM more often develop type 2 or 5 CRS, and patients with type 1 DM - type 4 CRS. The main criterion for the severity of CRS is the presence of albuminuria, not eGFR. CKD was diagnosed in 38% of patients with type 1 diabetes and in 67% of patients with type 2 diabetes. In patients with type 1 diabetes, an increase in the level of profibrotic cytokine TGF-β1 in the blood serum was noted before the appearance of albuminuria. When using glycosaminoglycans, a decrease in TFG-β1 levels was noted in the blood serum of patients with DM 1 type, however, the changes were more pronounced and were statistically significant only in the group of patients with newly detected CKD stage I. A2. The greatest decrease IL6 levels were significantly reduced in patients receiving combined therapy, especially the combinations of metformin + insulin and metformin + iSGLT-2.
