The conducted studies have shown that metabolic and immune processes are impaired in patients with COVID-19 against the background of type 2 diabetes mellitus (T2DM). Increased levels of bradykinin, AMPKα and IRS-1, activation of pAkt and a decrease in ERK1/2 were found, which indicates dysfunction of signaling pathways associated with glycemic control and the inflammatory response. In patients with acute COVID-19, the level of IL-6 increased almost threefold. In patients with diabetes mellitus who had COVID-19, a decrease in STAT3 in blood plasma was observed, which increased under the influence of insulin therapy. In blood mononuclear cells of patients after COVID-19, IKK activation and IκB reduction were noted, indicating increased inflammatory activity. In patients with newly diagnosed type 2 diabetes or disease duration ≤ 5 years, a decrease in Ca and Mg levels was noted. Se content is reduced in both prediabetes and type 2 diabetes. Type 2 diabetes on the background of subclinical hypothyroidism is accompanied by insulin resistance, dyslipidemia and a decrease in Ca, Mg, Zn and Se levels. Analysis of the intestinal microbiota showed a decrease in diversity and an imbalance in the Firmicutes/Bacteroidetes ratio in obese patients. Atherogenic dyslipidemia was detected in patients with overweight and non-proliferative diabetic retinopathy; with TG >1.7 mmol/l, the appointment of fenofibrate is recommended. Patients with the debut of type 2 diabetes after COVID-19 have been shown to have leukocyte changes and increased inflammatory activity, corrected by metformin.
