Limited specificity and high individual variability in responses to the same rehabilitation interventions have been shown. The persistence of the tension of adaptation mechanisms in the rehabilitation process was revealed, which indicates the insufficient influence of standard programs on the deep mechanisms of recovery. The need to revise unified rehabilitation approaches was substantiated. Conceptual principles for improving the rehabilitation of the population affected by military actions have been formulated based on the biopsychosocial approach and the principles of the international classification of functioning (ICF). The transition from symptom-oriented and unified interventions to personalized, functionally-oriented rehabilitation programs is justified. Functioning, participation, and adaptive capacity are identified as key rehabilitation endpoints. The translational potential of the proposed approaches for practical healthcare is demonstrated. An information and coordination model of rehabilitation has been developed, aimed at ensuring the continuity of the rehabilitation route. The role of the coordination link in increasing the coherence of interdisciplinary interaction and minimizing gaps in the provision of care is substantiated. The possibilities of using the model for optimizing the management of rehabilitation processes in the conditions of the war and post-war period are shown. Product Description popup.authors Oksana V.
